Penile Implant Surgery

Why Penile Implant Surgery?

Penile implant surgery offers a groundbreaking solution for men grappling with severe erectile dysfunction (ED). These surgically implanted devices have been transforming lives since 1973, providing a means for men to achieve rigid erections when other treatments have fallen short.

Many of these individuals have previously explored oral medications like PDE-5 inhibitors (such as tadalafil and sildenafil) without success. Additionally, penile implants can also be a valuable resource in the management of Peyronie's disease when accompanied by ED and curvature. Remarkably, the satisfaction rate stands at an impressive 90% for men and 95% for their partners.

In the words of one patient, "The control and quality that you want, the pleasure your partner will appreciate! I feel like myself again!"

When should I consider penile implant surgery?

Penile implants are typically recommended when other oral treatment options have yielded no results. This surgical procedure offers a permanent solution to erectile dysfunction, providing a reliable means to regain sexual function.

Elements of a Comprehensive Medical Evaluation

What elements should a full medical evaluation include?

A thorough medical evaluation is essential for determining the suitability of penile implant surgery. Key components of this evaluation may include:

  • The international index of erectile function (IIEF) or other validated questionnaires to objectively assess the severity of ED and postoperative outcomes.

  • A combined injection-stimulation test.

  • Penile color Doppler sonography with intracorporal pharmacologic injection.

How the Internal Penile Prosthesis Pump™ Works During Sexual Activity?

During sexual arousal, the patient can effortlessly trigger an erection by simply squeezing the pump positioned between the thumb and index finger. The results are so natural that many patients report their partners couldn't even tell an Internal Penile Prosthesis Pump™ was in place.

Our minimally invasive surgical technique allows us to conceal all the components of the penile pump within the scrotal sac, with the incision discreetly hidden along the midline raphe of the scrotal skin.

To end the erection, the patient merely holds the pump, releasing the fluid back into the reservoir and returning the penis to a flaccid state. This system empowers the patient to control the duration of the erection, ensuring satisfaction for both partners, even if a patient reaches climax quickly.

What to expect during the penile implant treatment process?

Before Penile/Penile Prosthesis Surgery:

Medical History: Your healthcare provider will conduct an in-depth inquiry into your medical history, including past and current health status. Information about previous surgeries, medications, and prior treatments for erectile dysfunction will be gathered. Rest assured that all your answers to these questions will remain confidential.

Physical Examination: To determine if a Penile/Penile Prosthesis is the most suitable course of action, you will undergo a comprehensive urological examination. Patients considering a prosthesis should not have an active skin disease, particularly in the genital area, before surgery.

Before the operation, patients opting for penile/penile prosthesis surgery should consider:

  • Managing blood sugar levels for diabetic patients.

  • Ceasing smoking.

  • Discontinuing blood thinners under medical supervision and, if necessary, adopting an alternative medication recommended by a physician.

  • Upon hospitalization for the operation, intravenous antibiotics will be administered to prevent pre-operative infections, and the genital area will be meticulously cleaned immediately before the procedure to minimize infection risks.

Penile/Penile Prosthesis Surgery:

The surgery can be conducted under general anesthesia or regional anesthesia (spinal anesthesia). The choice of anesthesia type typically depends on medical requirements, patient preferences, and the decision of the surgical team, which includes the anesthesiologist and the operating physician.

The duration of the surgical procedure typically ranges from 45 minutes to 1 hour. After the operation, patients may have a urinary tract catheter and surgical drains in place. The catheter is usually removed the following day, while the surgical drain is taken out within 2-3 days.

The maximum hospital stay is limited to 2 days.

Upon discharge from the hospital, patients will be provided with antibiotics and pain relievers.

During the recovery period, the penile/penile prosthesis remains semi-inflated and continues in this state until one week after the initial surgery. At the one-week follow-up appointment, the doctor will instruct the patient on operating the pump, which you'll use to inflate and deflate the device leading up to the day of sexual intercourse.

Athletic suspension panties are to be worn for a period of 10 days.

Patients can typically take a bath 2-3 days after the operation, and the stitches do not require removal as they will naturally dissolve.

Mild pain and swelling in the penis and scrotum may persist for 1-2 weeks after the surgery. Heavy exercises and lifting should be avoided for 6 weeks following the procedure.

Sexual intercourse is typically permitted 4-6 weeks after receiving a penile prosthesis. Ejaculation and orgasm are experienced similarly to physiological conditions during intercourse with a penile/penile prosthesis.

Types of Penile Implants

Penile implants come in two primary forms, each with its unique characteristics:

Flexible Implants (Coloplast Genesis):

These implants consist of two bendable rods that are inserted into the erection chambers of the penis. They offer flexibility, allowing adjustments during sexual activity. However, the penis remains semi-rigid, which may pose challenges in concealing.

Inflatable Penile Implants (Coloplast Titan Touch Bioflex):

These implants are fluid-filled devices, comprising two inflatable cylinders placed within the penis's erection chambers. A hand-controlled pump resides in the scrotum, along with a reservoir for storing fluid when the penis is not erect. To initiate an erection, the patient squeezes the pump multiple times, transferring the fluid from the reservoir to the cylinders. The same pump can deflate the device after use.
